PDC Information/Activity Request Form

Green Form

The first step in acquiring PDC points is to complete the green form, which can be found in both faculty workrooms.  If it is easier for you, you can download the attached form.  It is a fillable PDF, so you can type on the form and print it out.

PDC Activity Request

This form must be completed and approved BEFORE you attend the activity.

Whether you use the paper form or the internet form, you will complete the same steps:

    1.  Write the goal from your Individual Professional Form that the activity applies to.

    2.  Check only ONE box under "District or Personal Goal" OR "Service to the Profession" -- whatever type of activity it is.

    3.  Check which type of verification you are attaching to the form.  (You must attach verification of the activity.)

    4.  Talk to your principal about the activity and get his signature as well as the signature of your PDC building chairperson (Carol Dixson or Wendy Scott.).  

    5.  Finally, the form -- with the attached verification and all signatures -- goes to Brenda.
